Need simple donation report

I created a very simple donation page that I will link to from our organization's main website.  I need a report that shows donor name, email address, donation amount and donation date.  I can't get it through Square reporting, and the orders report through Weebly seems to create two rows for each order.  Any suggestions?

I think the best option is to run a Transactions report and filter by Source: Online Store. Hit the Export button in the top, right corner to download as a CSV to view customer details. 

Note: there isn't an option to further filter sales to view only donations.
