Have you tested any other Bluetooth devices to see if THEY can connect to your phone?

That sounds frustrating.
Here are some troubleshooting steps that may be helpful.
If you see Failed to Pair when attempting to pair a Reader:
Force quit the Square POS app.
Go to Settings > Bluetooth on your device.
Find your Reader and click Forget.
Reopen the Square app and re-pair at Settings > Card Readers.
