In Square Appointments, it always shows that one person booked the appointment, regardless of which user entered their passcode. Can this be adjusted so we can be certain of who actually booked the appointment?

I am a little unclear on your question. Is the appointment showing the same employee or customer each time it's booked?

We sign in to the Appointments app on an iPad with a single email address and password. Once signed in, two different individuals have unique passcodes to access the app (e.g. Bob & Mary). In this case, Mary is the business owner, and Bob is an employee. If Bob uses his passcode to access Appointments, it says Bob in the bottom right corner, and if Bob schedules an appointment it will still say that it was booked by Mary.

Thank you! Double check the settings for that particular employee to make sure they have access to Appointments and having those booked.

My business is made up of four people as of today.
When the customer services associate enters their 4 digit passcode to access one of our shared access points (iPad > Appointments app), it shows their initials in the bottom right corner. When they book an appointment, it shows as “Booked by the Business Owner” instead.
I’ve included a screenshot of their Permissions.
Please let me know if other screenshots or adjustments would be helpful.
