Troubleshooting CSV errors can be a bit tricky.
From what you've both described, it sounds like it might be the way the quotations are being recognized. Excel can read CSVs with quotation marks incorrectly sometimes. Check to make sure quotes haven’t separated a name into two different cells.
Extra commas can cause these errors as well.
