In Square for Restaurants, what's the best way to enroll a new customer in the loyalty program if we're using printed credit card slips for table service? The loyalty prompts will appear when someone signs on the screen, but I wasn't sure how it would work when you were using the paper version.

I ran your post by our Restaurants folks for more context. There isn't a way to do this with printed receipts right now as the customer can't note their phone number for entry.
For now, the best workaround would be to take payments at the table. With that, your customers can enter their information on the spot. Learn more here: Take payments at the table using the new Square Terminal with Square for Restaurants!
