Is it possible to have one widget that shows the store's physical location with one set of prices for services and another widget for mobile appointments that need customer address with a 2nd set of prices for same services?
Perhaps you could list each service twice, but name and price them different. For example: "1hr Massage - Come to Us" and "1 hr Massage - We Come to You"
You could list a fixed price for the on-site version and list a "starting at" price or even just "call us" for the off-site option, assuming you need to know their address or something before you can give an accurate quote.
If you're asking for some widget where it will give them a quote automatically based on them entering an address, that's not available directly through Square Appointments. Perhaps one of the alternate scheduling apps that can integrate with Square gives such an option, but I'm not aware of the details of those apps. You can find a link to Apps in the Dashboard main menu, almost near the bottom.

@RRKC There isn't a specific widget that would accomplish this, but there a few workarounds like the one mentioned above that would accomplish what you're trying to do.
I've also seen sellers use the ''Staff'' members as locations rather than actual staff members, so when your customers book with a specific Staff member they'll really be choosing their preference of Mobile or On-Site.
