I received a message that I needed to have one of my repeat customers fill out a "Repeat Payment Authorization Form." I have tried to open the form to get signed by my customer with card ending in ****, but it gives me a failure notice.
Can you please send the form via email and any instructions that I will need to follow to have my funds released to me.

I think this is the form they are referring to, you will want to make sure.
You can find this for under any customer in the dashboard. Goto Customers/select a customer/card on file/add card/click download.
