How do I get my contact us form to be active so customers can input their info and I receive the form info through email.

I am having trouble with the contact form.  I have 2 drop down lists for potential customers to choose 2 different services they would like information on.  Only 1 is showing up in the email I receive. How do I get both to show up in the email.

How did you set up these contact forms initially? If you and I can retrace your steps, we can find a solution together. 

Are you missing the other form when the make am appointment or somewhere else?
