Can we add automatic consult forms to square appointments please

Automatic sending of consultation forms for treatments is a very important feature that we all need (in the beauty industry).

We are suppose to have consultation forms stored for every client.

It would be amazing to have a feature that automatically sends out a consultation form that’s relevant for the booked treatment that the client fills out before they arrive to the salon. Sometimes they can’t have the treatment for whatever reason and then you have wasted everyone’s time. That could of been figured out if they had filled out a contraindications form prior to the day of treatment. Also no one wants a filing cabinet full of forms in their salon.

If anyone has a good recommendation to an app that sends out and stores forms in the mean time that would be amazing.

Thank you

Hi @BeautyBAE thanks for reaching out with this question!

The ability to automatically send digital customer contracts to your clients when they book a particular service is available via the Appointments Premium plan.

If you're not subscribed to this plan, you still have the option of creating, sending and storing digital customer contracts, however, these must be sent to your clients manually (jump into the below link for steps). Once set up, you can view all emailed and signed contracts through the Contracts page on your online Dashboard.

Visit this link to view how to Send Digital Customer Contracts with Square Appointments.

Hi Laurie,

Thank you for getting back to me. I had a look at the contracts before asking my original question and it just looks like something the client can signs to agree.

In the beauty industry you need them to tick boxes of products/medication that they are using/not using etc.

Is that something they can do with these “contracts” and I’ve just not looked noticed??

Hi @BeautyBAE,

You are correct, apologies for the oversight on my end! I've just tested this on my account and there isn't a way to include a question and response section via Contracts.

It does look like we've responded to a similar request in the past here, where we suggested making use of the Contact Form in Square Online and/or Google Forms.

Have a read through these options and let me know if any of these may work for you. In the meantime, we'll continue to pass on this feedback to our product team.
