Not doable but you can add a notes section to the checkout screen. Can you tell me how you would use the feature you're asking for so our Product Team understands what you're looking to accomplish?
So my client is selling medical supplements, and the shipping method is pickup only, so they asked me to add a fixed preferred pickup day & time, so they can prepare it on that date.

Ahh, I see. Unfortunately Square Online doesn't support fixed dates for pick up. I supposed you can apply a modifier set to your items named Pick up Dates with the time slot options you offer. That will provide a drop-down menu-esque moment you're looking for when customers select the time for purchase.
