We are a sports coaching business. When parents register their children for one of our sessions I need them to be able to provide the child's name, gender and date of birth at checkout. Is there a way to add this into the checkout form?

The first idea that comes to mind is to create a text box modifier and add this to your item or service that requires this additional information from the customer. This will prompt your customer to enter the required information before being able to complete their payment online. Ensure when setting up your text box modifier, that you tick the option to Make this a required field.
You can name the label of the text box to suit your needs as well, for example, Enter Child's Name, Gender and DOB. See my example screenshot below of what this will look like for your customers at checkout.
