I've had 2 appointments that have been cancelled/no showed and I have a 24hr policy and hold cards, I've yet to receive that cancel fee.

Did you mark these appointments as no-shows on your calendar? Once you do this it will charge the customers card on file.
Before when I marked at no show nothing happened, when I did cancelled it all do I want to charge my cancel fee I clicked the toggle to yes and nothing happened

When you tried to mark an appointment as no-show did the clients card decline?
