I found an old post for this, but it doesn't reflect changes to the system.

Is there a way to add a note field to the checkout form?

I can't find the actual form or where to select fields for the form

HI, you can find the setting under Checkout.

Here are the breadcrumbs and screenshot to find.

Online Dashboard, Settings, Checkout, far right click Advanced Settings, Save
