Invoices: How do I request a customer's shipping address?

I am new to Square.com.  I see that there isn't a place to add shipping cost other than adding a line item.  I tested an invoice and selected the request shipping address but the invoice I recieved did not request a shipping address?  I see that this was a known issue.  Please advise.

If you opted to collect a customers shipping information, the fields for the customer to fill out will be displayed when they make a payment. 

When you clicked on the payment link, was there no option for shipping information underneath the credit card details?
