How do I obtain information from my customer?

I run an eyewear business and in order to process sales for my customers I require their Pupillary Distance and a copy of their prescription. How do I configure Square to allow for the input of this information during purchase on my website?

Our contact forms are not HIPPA compliant so you would need to find a third party option that would be compliant.

Once you do, the next step would be to use the Embed Code option to add it to your site.
