How do I get a free reader from a retailer?

Is there a code or voucher that I need to print and present to the retailer in order to be that free square card reader?

No. I just did this an hour ago. Go buy it. Open it. On the last page of the booklet inside you will see a code to redeem. Go to square.com/redeem and enter the code. You will then have $10 deposited into your bank account. Have not seen my $ yet but that is the process described.

Keep in mind your first magstripe reader is free when you sign up, and additional magstripe readers cost $10.
