How do I add city, state, and zip fields to my shipping address inputs for customers?

When a customer buys my item online, they have to put in a shipping address, but there are no input fields for city, state, or zip.  How do I get them added?

Do you have the item set up with the shipping option? And how do you have your shipping set up for your site?
