Customizable checkout form

I saw a previous thread from years ago that the checkout form is not customizable. Has that changed, or is there anyway to collect additional information before checkout? We need to collect DOB, ethnicity, gender, and address. 

For our service we are required to collect that information by the State.

@pcappelo thanks for taking the time to post and welcome to the Seller Community. This is still a feature request to make this information required at checkout, but you can always add a customer to a sale and add this information at the same time. Once you create a customer (this can be done at the checkout screen), their information will be saved for when you add them to a sale in the future.
