"Deposits Suspended"

Why is my account "deposits suspended", and requesting information about my business that I cannot provide? I'm a sole proprietary day care, and I limit my case load below the threshold requiring State certification. 

I set up my square account and attempted the first transaction with a child's parent. The amount was taken from their card but has not appeared in my "balance".

Help...
