This would be accomplished via the Team App's Request Time Off feature.
From the Team App:
Tap the Request time off button, select the time frame you want to request off with the date selector tool, add an optional note, and tap Request off.
Note: This feature is only available to Shifts Plus or Team Plus subscribers.
