can i upload a pdf fillable form that can be emailed to my patient to fill out

i have a teeth whitening business. i would like to upload an already created fillable form , have it emailed automatically to patients when they make appointments, have them fill it out and email it back and save under their account is that possible. I only see that I can create a whole new form from scratch

What you have in mind would require a bit of custom programming because you want a specific work flow.   You would probably have the same challenge with other site hosts that have a similar type editor setup and out of the box functions.   There's maybe two options - find a Developer who can work with you or use a 3rd party App or service that has the features you need.  Square has a Developer Forum and they also have Marketplace for Apps that are compatible with Square.    You can go outside the box and consider a different process.  Check out Jotform - you can create a custom from from scratch or modify an existing template for your needs.  Once a customer or client makes an appointment, you manually mail the form to them - it's not an automated process but, a work around.  Another thought is to use a Cloud Service like Dropbox or Google or Microsoft Business - you keep the forms stored there and send a link to the customer or client - they click the link, go to their form fill it out and you have your data (all stored in the cloud).
