Custom Questionnaire/ Client Intake Form

Hello,

For those with Square appointments Only, we need to be able to build a custom questionnaire/intake form when client's are booking the appointment. To prepare for my clients appointment, there are 10 custom questions I need answered.  The supported form integrations are not working for those with Square Appointments Only.

This feature has been requested multiple times. Please listen to the seller community.

This would be a fantastic feature especially if the inputted data made it back to the Customer/Client Profile for easy reference!

Thank you guys for sharing your thoughts on this! Not sure if this is exactly what you are looking for, but using Square Appointments on any pricing does allow for the option to send digital customer contracts. @Ellie_ Contracts and even Invoices are great features that us Appointments users utilize.  But what we are looking for is more of a Forms type feature.  Contracts is like an agreement: info is laid out and the customer intials.  But with some type of Form, we can ask questions related to the situation.  Estheticians need info on someone's skin care routine and allergies, hairstylists/extensionists need questions answered about prior use of extensions or haircolor.   Right now, many of us have to use a 3rd party app like Jotform or Google Forms but all this info lives separately from Square.  We would love to have this info be a part of the customer's file, so it can be referred to seamlessly and efficiently.

I would love to have this as well. I currently use Jotform but if it was built into square and would add them to my client list with their info that would be much more streamlined! 
I currently have Jotform set up that after they complete the form it takes them to my booking site to schedule online. But then I have to go back and enter their address and info to their file.

I am also an esthetician and would love this option! I currently use Jotform as well, but it would be so nice for Square to offer it and automatically send it out for specific services booked or first-time clients. Vagaro and Glossgenius both have this feature already. I love Square, but I would be lying if I said I hadn't thought about switching because those company cater to the beauty industry more than Square does. 

I have been looking through the Seller Community posts and this has been requested by so many people for so many years! Let's get this prioritized, please!

This feature would be so beneficial. All information gathered and saved to the customers profile. Please add a questionnaire option.
