Custom Questionnaire/ Client Intake Form

Hello,

For those with Square appointments Only, we need to be able to build a custom questionnaire/intake form when client's are booking the appointment. To prepare for my clients appointment, there are 10 custom questions I need answered.  The supported form integrations are not working for those with Square Appointments Only.

As a Nail Tech I would really appreciate this feature. I wanted to switch from Acuity to Square and this is the one item that would secure my confidence in square!

We definitely need this!!!

Having the ability to integrate forms (not Contracts!) would make this Square compete with other POS applications on a different level.  Not having this feature causes us to start looking for other solutions.  We do have forms on Cognito Forms (and have used JotForms as well), but as others have mentioned, the form data lives outside of Square and we really need this integrated with our customer record.  For example, customer books an appointment, but we require two forms prior to proceeding with the appointment:  Medical History Intake Form, Treatment Consent Form.   I have tried using Square Contracts, but it's not the same thing as an intake form.  Also, the Signatures are not real signatures and even if you use the Contracts, they don't get linked to the Customer.  You have to go back to the Square Contracts section to find them.  It's useless for our purposes, and it's not a "contract" we are requiring.  Rather than re-inventing the wheel, if Square allowed integration with the two leading platforms: JotForms and Cognito Forms that would help so much.  Currently the only integration is from a payment collection standpoint via the 3rd party platform, but we wish to keep it all in Square.  Many POS/Appointment applications have Forms, but frankly, most don't do forms as well as JotForms and Cognito Forms and don't offer advanced features that these platforms support.  Please consider this much needed feature.

Square doesn't care about their consumer's needs. This request was posted two years ago and has 13 "likes" and dozens of comments. We are still requesting this feature after two years. This is one of the reason I will be looking into leaving Square and would encourage others to do so, until they listen to us as small business owners.

That's really disappointing.  😥  If they did even a little research they would see that most platforms offer forms.  Even in Appointments/Settings/Communications options there is a spot to "Add a Form" but, it's not really a form - you can only add a Contract.  Contracts are just not useful at all.  Thanks for the reply.

As a massage therapist and esthetician, I believe that integrating custom forms (not just contracts) into Square would greatly improve my workflow. Currently, I use another type of form, but the information remains outside of Square, making it difficult to keep everything organized in one place.

When clients book appointments, I require two forms for massages—a Client Intake Form and a Liability Release Form—and three forms for facials, which include the two massage forms plus a Skin Analysis Form. Unfortunately, Square Contracts do not provide the necessary features, especially regarding electronic signatures and linking everything to the client’s notes.

Having this feature available directly within Square would make a significant difference, not just for me but also for many others in the community who share this need. Thank you for considering my request!
