Refund to a different transaction

I recently issued a refund to a transaction.  Today I was notified that the transaction after that was also automatically issued the same refund.  Why?

@redware1 Not sure, I know when you do a refund it shows as the original transaction and then the refund transaction.  You can go to the transactions tab and see who did the refund or you can go to reports (online dashboard) and go to activity log and drill down into that report.

When you say notified, just curious if you have reviewed and looked at the transaction.  Usually what the e-mail goes out as "You have requested a refund" and then you will get a 2nd email that says  "You have refunded".  But it is still for the same transaction.
