I'm scared to proceed further because I went through twice and got rejected, but how do I get my identity verified? I'm sure I'm typing in my information incorrectly. I don't want to waste my remaining tries.

Well First I would suggest typing in the information correctly.  If you are not sure what information the question is asking for, I would suggest calling Square Support as you are trying to verify your information.  This way they may help point you in the right direction, where to location the information you need to enter.
