QR code to register themself as loyaloty member.

We are going to implement square and i want to achive below with loyalty.
1. customer to give out QR code to register themself as loyaloty member.
2. that customer registartion page should link to Customer registration page that we do manually with all the details.. name, phone, email, and option to upload they identification. as part customer type they belong to .. Student- studnet card, Senior citizen - sernior citizen card etc etc..
this is something i need to urgently and desperatly .. i need solutions.. Please help
