No Show & Cancellation Policy

I am running into an issue with some of our appointments having a credit card on file and some that do not.  ALL of our appointments require a credit card in order to book online.  If we are not editing the appointments, or deleting them ourselves, why do some of the appointments have a credit card on file and others do not? I've read thru some of these threads, and it seems from what is being said, credit card info should always be attached to an appointment. This is where I am confused.....
