I am wondering if a customer has to enter their phone number every time they make a purchase, or if the program will recognise their credit card and sign them in automatically following their initial sign up? Also, is there a way for the customer to check their balance themselves?

Your customers would need to enter their number to sign into their loyalty account, and it is not linked to a particular credit card so customers can use different payment methods and still collect loyalty points.
