I am selling Event Tickets via my Square Website but I require attendees to give consent information at the same time.
I have been able to create a consent form but this is not linked to a purchase.
Has anyone worked out a way of adding additional fields to the basket or checkout screens?

Hi @fearless24, thanks for posting and welcome to the Community 👋.
If the consent form you created is a sharable link or a file to download, you're able to customise your order confirmation email to include the form.
To get started, you'll first need to create a Sender Profile in Communications > Email and Text Alerts. Once you've verified your email address, click on Edit Email under the email type you want to edit.
You can also add additional fields at checkout. However, it is limited to adding a customer note and opting in for marketing, so I'm not sure that it would fit your requirement.
Let me know if you've set up your consent form differently, and I'll be happy to find an alternative solution.

Thanks for the suggestion. It's more that I'd want the information before completing the purchase rather than allowing them to do it after. However, if I'm unable to do this your idea is very good and I will definitely be using this as well.

I'd like to do some tests on my end to find a workaround, or see if we have an integrated partner that could facilitate this @fearless24.
Could you give me more details about what this consent form would look like (how many fields, does the customer need to tick a box etc.). If you've already created the form, you can add the screenshot in this thread.
