Consultation Form - Appointments and POS App Integration

We are needing to capture information for clients that visit the shop for barber or salon services. For instance, when a customer comes in, we need to ask numerous questions regarding COVID. Then that client might be doing a straight shave service - and for both, we would need them to sign off on the information they've provided or terms acceptance. 

Additionally, for salon services, there is the need for the colourists, to have a consultation with the client and go over the required information and set out the treatment required for colouring: tube, amount, etc. 

For everything, we want this to be captured within the Square / App system so that the info can be brought up and referenced at future dates.

We've looked at JotForms and Waiver Master but both are more sided towards being a payment system - which we don't need as we already have that in place via the POS or Appointments apps.

We want to be able to look at a customer in Square and pull up that customer info.

Any suggestions?

Hey @JPWilkinson,

At the time we unfortunately don't offer anything like this. We have had many sellers who have started to use JotForms and they're happy with what it offers but we don't offer anything like this at this time.

We're aware of the requests for it though so hopefully this is something we can offer in future improvements.
