The weebly online store has forms you can create to ask the questions you need answered.
You create a new page on your Weebly site, add a section, scroll down to the bottom of the add section panel and select forms. You can choose a basic template, then add/eliminate questions as you need.
There are lots of types of question types you can use: email format, phone, date, time, multiple choice, address, short/long answer.
You can set a different email for each form to submit responses to, or you can see them in the Forms Submissions section on the dashboard or on the weebly app.
You can link them to the form page from your website, or just send a link in email/text.
