Appointments / reservations for restaurants

I am using appointments for reservations. Would be nice if I could add an extra line for them to fill out asking them how many people they had coming.

We could use the same type of request to schedule appointments for wine tastings - they're generally as a group of unknown size so we need a bit more flexibility with this.
