When a card is read, where does the user name come from. Is it read from the card or does it come from square?
Comes from the Card which includes the Account Number, or the 16-digit card number (or 15-digit for American Express), Expiration Date, Cardholder Name, and Service Code. If you're into details, there's some good stuff here.
