What is the actual name/address for Square Banking?

I am filling out a customers sheet for them to make ACH payments.  Does anyone know the name and physical address for Square Checking?  Not sure if it is actually Square or not.

I don't believe this information is needed in order to send/accept ACH Payment Transfers. How are you accepting these payments? Are you doing it through Square Invoices or something else?
