I need to be able to accept special orders online, with 3 different options

Hi

I make and sell unique reversible Scrub Hats. 

Whilst I have a stock of pre-made however my Main Point of Difference is that I can custom make with my stock fabrics.

I need for them to:

1. select a size (there are 3 options)

2. select a fabric design (there will be 20-50 numbered #1, #2 and so on), and I need to be able to display the fabrics - ideally in the dropdown, or as a part of a photo array with the #s.

3. select another fabric design from the same options as above

I am hoping there is a simple solution for this?

Also in addition to this I would like to offer embroidery at an extra cost, this would require a free-text box for the customer input.
