Taxes are NOT being applied at checkout

This is my 3rd time trying to resolve this issue. Getting resolution on this platform is proving to be extremely hard. I will try this last time and then be forced to move to shopify if I am not able to resolve.

Tax rule is setup and applied to all items on my website however when customers check out no tax is applied. And sometimes Shipping is not applied either.
