Estimated Shipping is wrong

I have a Canadian online store and I've removed all shipping profiles except Canada in order to streamline. I have $18 flat-rate shipping and free shipping over $99 Canada-wide.

1. I was hoping that would activate the "Free Shipping Progress Meter", but I haven't been able to do so.

2. I was also hoping it would fix the bug where the cart shows the wrong "Estimated Shipping" amount. It still displays $0 until the customer enters their address.

What it should do is:

A. Progress bar at the top of the store that starts at $0 and at $99 says "you've unlocked free shipping"
B. Estimate shipping at $18 until the customer has reached $99 regardless of whether they've gone through the checkout process to enter their address. This can be assumed because the only shipping addresses will be Canadian.

If "B" is impossible then Square should at least allow the Estimated Shipping section to be hidden until the customer enters their address.

These are all e-commerce standards at this point. Has anyone been able to fix these with their own code?
