So I need to know if you cancel someones appointment and add a note where it says "client note" to the cancellation telling them why you're cancelled, do they get that message on the automated text?

Thank you for posting in the Seller Community. Welcome.
After looking into this with our Appointments Team. I can confirm that your customers don't see the note entered on your side. The client note is strictly for the sellers side. When you cancel an appointment it gives the option to send a message if you have email notifications enabled.
