Declined transactions on Square register

Hi there , 

Have been using Square for a week and have found several transactions declined on the Transaction status page 

we are using the Square register , so no online , card payment is taken then and there , 

How can this happen ? I have got all staff to check that a blue tick approved is done before letting customer leave , Can it be declined after the sale ? and does this mean that they have not paid for the product ?
