Is there a way for me to add a medical history form that the client needs to fill out at the time they are booking their appointment? I do not want to send it to them after they have booked it, I would like them to fill out this form in order to make a booking. In context, I am a tattoo studio and would like to know the type of tattoo they would like before they have booked their appt.
