Custom Intake forms or terms agreement

I like how you can have a contract sent to customers and customize them.. However, ours are dependent on whether their appointment is accepted or not and if they do not sign the contract, their appt is not accepted.

It would be GREAT if we could just place a terms or policies agreement requirement right on the booking page!! That way they have to agree to it right there (just like the cancellation policy) and we don't have to worry about sending contracts.

Aquity does it, but so far, I do like square better! 
This would be super helpful for my business!

Intake forms has been a long-time feature requests. 

It is probably the number 1 feature request for Square Appointments. 

Hoping it is addressed in the next year.
