Using query string to pre-fill form field

I want to create QR codes with unique codes that get put into hidden fields on a form.

Example:

URL http://my_domain.com/contact_form?secret_code=1234

will land on my contact page with a hidden field named "secret_code" is filled with the value 1234

How do I do this?
