Upload form to Square Appointments Dashboard

Has anyone been able to upload an existing form ( I own a spa clients fill out forms on their 1st visit) into the Square appointment dashboard? I know Square appts has a way to create a form but it's very simplified and my state board provides forms for liability protection we should use. Thank you for any feedback on how to go around this

Use Square's Contracts feature as a workaround to create your detailed intake form. Here’s how:

  1. Create a Contract Template: In your Square Dashboard, go to Contracts and create a new template. You can build out sections that include everything your state-mandated liability form requires. Add text fields, checkboxes, signature, and initial fields for clients to complete each section, like health history, consent, and liability clauses.

  2. Automate Sending: Set it up in Square Appointments to send the form to new clients automatically upon booking. This way, clients receive it only once and it’s saved in their profile.

  3. Restrict to One-Time Completion: Once completed, it’s saved, so clients won’t get repeat forms unless you manually reset it.

This approach allows you to make your required intake forms available in Square, ensuring compliance with your state board's standards. Let us know if you have questions! 🙂

Currently, Square Appointments supports form creation directly in the dashboard, with the option to create a custom template. But I  understand that the customization options can be limited, especially when you have specific state board forms for liability.
