How do I add anothe rtext box

I am trying to edit an existing form. I need to add an additional category but it is being rejected. I keep getting a message that says I need the Pro Package to add more than five fields. However, it alreayd looks as though the form has more than five fields (unless I am counting them wrong). New to this - inherited from previous employee.

It looks like the site was upgraded in the past and I would guess that was when the extra fields were added. In order to add another you'll definitely need to upgrade again, though.
