Has any one created a mail-to form that is required when purchasing a specific item. The scenario is selling memberships and the mail-to form would capture demographic and other information, then on checkout who be emailed to the Registrar for entry into the member management spreadsheet.
I'm not aware of anyone doing something like that - I think it'd be pretty complicated to try and do something like that. You could sell a membership product, then provide a link to a hidden form to the customer after purchase.

Oops - I didn't get as in-depth as I probably should have. I was thinking something like a digital product which included a PDF; this PDF could have instructions along with a link to the form. You could even go a step further and make it an editable PDF that people could upload to the form.
