I would like to have a form that works like this below. I do not really want to build three forms on three pages and have to have another page to direct people to each type of form. That's what I want to avoid. Can I have the whole form behave based on the first drop down?
First field is a drop down with three choices {website vistor, in person visitor, regular}
If the first choice is selected the rest of the fields look like this:
Name
Phone Number
Comment
Submit
If the second choice is selected the rest of the fields look like this:
Name
Spouse
Address
City
State
Zip
Phone
Children
Dropdown A (when did you visit)
Dropdown B (how many times visited)
Dropdown C (what are you interested in)
Other Information:
Submit
If the third choice is selected the rest of the fields look like this:
Name
Spouse
Children
Checkbox if this is an update
Address
City
State
Zip
Phone
Check boxes for best way to contact (email, mail, phone)
Check boxes for interests
Prayer Requests/Praise Reports
Checkbox to share prayer requests

It wouldn't be possible to do that with our own contact/survey element, though we do have some third party solutions in the App Center:

I don't know specifically if any of those do that, but it's worth checking to see.

It looks like two of the paid apps could do it. One calls it branching logic and the other calls it conditional logic. However, either are outside our very limited budget. As we grow as an organization, I may use one of those, right now, I'll have to build three forms on three pages, and then one page to link to those pages.
