Forms not working

I have a form on my site, but it is saying it has submitted but nothing comes through on email or in the website account bit.  It also allows things to be say they have been submitted even though requested fields havent been filled in.

Once you see the automated chat window try typing something like “speak with a rep” to be directed to the teams available to your account.
