How do I add and build a SCHOOL ENROLLMENT FORM page?

Do I have to choose an app? Which school-related form or doctor-related form do you like best? I want parents to answer quite a few questions, so the form will be much longer than a contact inquiry. Do have to use Weebly Education? I'd prefer Weebly, if you know how to do it -- thanks!

Hi there. We have an internal contact form option, but depending on the type of questions you need to ask it may be a better idea to use something like JotForm. They should have an embeddable option so that you can add the form to your site.
