Need help to make manual Medicare and Social Security payments

I goofed up, and I need to make one month's Employee Medicare, Employee Soc Sec, Employer Medicare and Employer Soc Sec payments to the IRS.  Square can't do it.  I've been on the EFTPS site to make the payments, but it's asking for a Tax Form Number?  Any guidance would be appreciated.

Hey @RUCS! Are you talking about your Tax Identification Number (TIN)? That number is issued by the IRS, so if that's the number you're needing, you'll need to contact the IRS.
