How can I add a custom field to Online Items?

We want to allow customers to add a personalized name to a sign item in our store.  How do I create a field in the Item to have them type in the personalized name?

HI, you would add a Text Box under Modifiers.

ex. Family Name (up to 20 characters)

How it displays to customer on website.
